360Health is provided by Teladoc Health Australasia Pty Ltd (ACN 147 387 666) (Teladoc Health). It is intended to provide general health information and advice. This service is available to you, your spouse, your parents, your parents in-law and your children if you are a member of GuildSuper. Teladoc Health is not part of Equity Trustees Superannuation Limited (the Trustee) or its insurer MetLife, and services are provided by Teladoc Health, not the Trustee or MetLife.
The recommendations provided by this service are based on medical and other information you provide to Teladoc Health. Teladoc Health is the entity that will collect, use, disclose, store, secure and dispose of your personal health information if you use their services. When you apply for insurance, you have a duty to take reasonable care not to make a misrepresentation to MetLife. The personal (including health) information you provide to Teladoc Health in connection with your participation in 360Health will not constitute disclosure to MetLife for the purposes of discharging this duty. MetLife does not have access to this personal information and any information provided by Teladoc Health to MetLife will be de-identified information only.
